financial peace of mind. Since premium payments are treated as medical expenses
under the Income Tax Act, there's potential for a tax credit. If you are self

Four Health and Dental Plans
A selection of four Health & Dental plans offers increasing levels of comprehensive
coverage for Prescription Durgs and Dental Services in addition to Core Benefits such as
Vision Care, Accidental Death and Dismemberment, Hospital benefits, Registered Specialists
and Therapists, Homecare and Nursing, Accidental Death, and much more.
Four Dental Plans
The Base Dental, Bronze Dental, Silver Dental and Gold Dental plans provide a range of
coverage for Dental Services. each of the Dental plans also includes the Base Core
Benefits like Vision Care, Extended Health Care, Homecare and Nursing, Accidental Death,
and more. A completion of a medical questionnaire is not required.
Important Notice
Medical Underwriting
All Bronze, Silver and Gold
Health & Dental Plan applicants will be medically
underwritten, meaning the premium charged and/or benefits offered could be subject to
adjustment or modification or coverage based on your medical background. This will
be determned after an evaluation of the information provided on the medial
questionnaire. Base Plan and all Dental Plan applicants are not required to complete
a medical questionnaire.
Acceptance Period
If your medical history is
such that a higher premium is required or that special conditions be applied to benefits
(see Medical Underwriting), you will be notified in writing prior to your decision to
accept the coverage. If at that time you decide not to proceed with the coverage,
your initial payment will be returned and your application cancelled.
Hospitalization/Pregnancy
Full coverage is
available for expectant mothers who apply within first 20 weeks of pregnancy (hospital
stay is limited to 2 days). Liberty Health cannot guarantee the availability of
private or semi-private accomodation.
Newborns
Newborns are
automatically added to your coverage if an application, with appropriate payment, is made
within 30 days of birth. If application is submitted after the 30th day following
the birthdate, medical information will be required.
Maximums
All maximums
are per person. Unused protion of benefits cannot be accumulated and added to
coverage in future months or years.
Effective Date of Coverage
Coverage is
effective the first day of the month following approval of the application.
